A range of forms, developed in conjunction with Dental Protection. Patients and dentists can use these forms to establish consent prior to treatment.

The forms include a checklist of issues. This allows a discussion with the patient and a record of the agreement and understanding of your patient.

The pads are supplied in sets of 50. You can mix individual forms to make up a minimum order of five.

A4 size NCR duplicate pad. Each set includes:

  • Cover Sheet
  • Divider card with general instructions for the dentist
  • A two part consultation record (patient and dentist copies). The patient copy has post treatment advice on the reverse.
  • A general information sheet  for patients explaining why the procedure is necessary (not required for Tooth Coloured Restoration & Endodontic Treatment)
